All our surgical patients and all active non-surgical weight loss patients have been fully transferred to the surgeons and physicians of their choice to continue their journey. Your new doctors have all your records from this office for continuity of care, and per your request and consent to share records.

Office notes have been already shared with your Primary Care Providers (PCP) after each visit. Therefore your PCP has your records from this office.
